SV Premier Care Ltd (“SV Premier”, “we”, “us”) is a domiciliary care agency registered with the Regulation and Quality Improvement Authority (RQIA registration 021048). We are the data controller for the personal information described in this notice. This notice explains what we collect, why, who we share it with and your rights under the UK General Data Protection Regulation and the Data Protection Act 2018.
Who this notice covers
- Clients and their families – people who receive or enquire about our care services, and their relatives or representatives.
- Job applicants and staff – people who apply to work for us, and our employees and workers.
- Website visitors – anyone who uses this website or submits one of our online forms.
What we collect and why
Clients. Name, address, contact details, date of birth, next of kin, GP and other health professionals, health and care needs, medication, care plans and daily visit records, and payment details for privately funded care. We use this to assess, plan, deliver and record your care, to keep you safe, to invoice for our services and to meet our legal and regulatory duties. Health information is special category data; we process it on the basis that it is necessary for the provision of health and social care (Article 9(2)(h)) under a duty of confidentiality, and otherwise under our contract with you and our legal obligations as a regulated provider.
Job applicants and staff. Contact details, employment and education history, references, right-to-work and identity documents, driving licence and insurance details, NISCC registration, training records, AccessNI disclosure information and, where relevant to the role, health information. We use this to recruit safely, to meet the requirements of RQIA, NISCC and AccessNI, to administer employment and payroll, and to sponsor workers where applicable. Criminal records information is processed under the Rehabilitation of Offenders (Exceptions) Order (NI) 1979 and the safeguarding conditions in the Data Protection Act 2018.
Website visitors. Information you enter into our contact, application, feedback and reference forms, and limited technical data (such as IP address and browser type) collected by our hosting and analytics tools to keep the site secure and understand how it is used. We rely on your consent for form submissions and our legitimate interest in running a secure website.
Who we share it with
We share information only where necessary: with Health and Social Care Trusts, GPs, district nurses, pharmacies and hospitals involved in your care; with RQIA, NISCC, AccessNI, HMRC, UK Visas and Immigration and other regulators where the law requires; with our care-planning, HR, payroll and accounting software providers, who process data on our instructions; with our HR and legal advisers; and with the emergency services or safeguarding authorities where someone is at risk. We do not sell personal information. Where a provider stores data outside the UK, we make sure appropriate safeguards are in place.
How long we keep it
Client care records are kept for the periods required by RQIA and Department of Health guidance after care ends. Staff records are kept for the duration of employment and for the statutory periods afterwards. Unsuccessful applicants’ details are kept for six months after the recruitment decision unless you ask us to keep them longer. Website form submissions are kept only as long as needed to deal with your enquiry.
Your rights
You can ask to see the information we hold about you, ask us to correct it, ask us to delete it or restrict how we use it, object to processing based on legitimate interests, and withdraw consent where consent is the basis we rely on. To exercise any of these rights, contact us using the details below. You also have the right to complain to the Information Commissioner’s Office at ico.org.uk or on 0303 123 1113.
